Many dental and GP practices pay staff monthly, so payroll ought to be routine. It rarely is. Employed and self-employed clinicians work side by side, staff can be on different pension arrangements, and part-time hours and extra sessions change every month. Payroll Service (UK) Ltd is a Cardiff payroll bureau that has run payroll for UK employers since 2011. We run the payroll for your employed team and give you the figures you need, so your practice manager is not spending the end of every month on it.
Why practice payroll is different
Dental practices and GP surgeries share some payroll issues and differ sharply on others, particularly pensions.
Who belongs on payroll
Dental associates are often engaged as self-employed, and GP partners share in the practice’s profits rather than drawing a salary, so neither normally appears on the practice payroll. Salaried GPs, practice managers, nurses, receptionists and other employed staff do. Where an associate is an employee, they are paid through payroll like anyone else.
NHS Pension Scheme in GP practices
GP practices holding NHS contracts are required to offer the NHS Pension Scheme to eligible employed staff. That means deducting tiered member contributions, adding employer contributions and getting the figures to NHS Pensions on time. We calculate them each pay run; NHS Pensions administers the scheme itself.
Dental practice staff pensions
NHS Pensions states that dental practice staff, such as dental nurses, hygienists, therapists and practice managers, cannot pay into the NHS Pension Scheme because they are not employed directly by the NHS. They are assessed for workplace pension auto-enrolment instead, using a scheme such as NEST.
Clinical and non-clinical pay
Salaried clinicians, nurses, dental nurses, receptionists and managers are often on different pay structures, from annual salaries to hourly rates. Each needs setting up correctly and paying the same way every month.
Extra sessions and cover
Part-time staff who cover extra sessions, late clinics or Saturday mornings need the additional hours paying at the right rate, and their pension deductions recalculating to match the pay for that period.
Absence in small teams
Small clinical teams feel absence quickly. Statutory sick pay is the lower of £123.25 a week or 80% of average weekly earnings, and maternity, paternity and shared parental pay need calculating and reclaiming through your EPS.
NHS Pension Scheme: what we do and what we do not do
For GP practices with staff in the NHS Pension Scheme, we set up each member’s pensionable pay and apply the member contribution tier and employer contribution rate that apply. Each pay run the deductions appear on the payslip, and you receive a report of member and employer contributions for your practice’s NHS Pensions returns and payments.
We are not NHS Pensions’ administrator. We do not decide who is eligible for the scheme or how pensionable pay is defined for a particular role; those questions sit with your practice and NHS Pensions. What we make sure of is that the figures in the payroll match the information you have given us, every pay run.
Staff who are not in the NHS Pension Scheme, including all dental practice staff and any GP practice staff who opt out, still need assessing for auto-enrolment like any other employer’s staff. We handle the assessment, enrolment and contribution files for schemes such as NEST, Smart Pension and The People’s Pension, at no extra charge.

Dental and GP practice payroll questions
Do associate dentists go on the practice payroll?
Usually not. Associates are often engaged as self-employed and paid outside payroll. HMRC withdrew its specific guidance on associate dentists’ employment status from 6 April 2023, so status depends on the actual working arrangement. If you are unsure, take advice. If an associate is an employee, we pay them through payroll.
Can our dental nurses join the NHS Pension Scheme?
No. NHS Pensions states that dental practice staff, including dental nurses, hygienists, therapists and practice managers, cannot pay into the NHS Pension Scheme because they are not employed directly by the NHS. They are assessed for auto-enrolment into a workplace pension instead, which is included in our per-payslip price.
Do you deal with NHS Pensions for our GP practice staff?
We calculate member and employer contributions each pay run and give you the figures your practice needs. We are not NHS Pensions’ administrator, so eligibility decisions, scheme queries and the practice’s own submissions stay with you and NHS Pensions.
Can you pay salaried GPs?
Yes. Salaried GPs are employees and are paid through payroll like other staff, with PAYE, National Insurance, student loan and pension deductions calculated each pay run. GP partners are normally self-employed and are not paid through payroll.
Can you run payroll for more than one practice?
Yes. Each practice with its own PAYE scheme has its own payroll and its own reports. Pricing is per payslip, with no setup fee and no contract.
What does practice payroll cost?
Monthly and four-weekly payrolls are from £3.50 + VAT per payslip, and weekly and fortnightly payrolls from £2.50 + VAT. Starters, leavers, P45s, P60s and pension auto-enrolment are included. See our payroll pricing.
Can you take over from our current payroll provider mid-year?
Yes. We take over from any payroll software or provider, at any point in the tax year, and bring your year-to-date figures across. Setup is within 24 hours of receiving the information.
Do you pay our staff?
You stay in control of payments. We give you the payment report for each pay run and you pay staff from your own bank account.
